  4. Last will and testament
  5. Written list of personal property intended for certain persons – separate from will
  6. Designation of person to arrange funeral
  7. Trusts – in certain situations
  8. Special needs trusts – for disabled persons
  9. Long term care planning/medicaid planning
  10. Long term care insurance
  11. Beneficiary designations on retirement funds, annuities and insurance policies – preempt beneficiaries named in will – name primary and secondary beneficiaries
  12. Record of all investments and assets, names of owners, and beneficiary designations
  13. Title on all assets consistent with estate plan
  14. Guardian for minor children
  15. Location of key for safe deposit box (if have one)
  16. Keep written record (on paper) of passwords, user names and answers to identification questions
